Patient Information

Please arrive 15 minutes early to complete your intake paperwork, unless you have the first appointment of the morning or first appointment after the office's hour closure for lunch.  Please also be sure to bring the following items with you to your first appointment:

• Physical therapy order or referral – For physical therapy from your physician if you were seen by a physician

• All insurance information – Necessary to process your claims

• Driver’s license or state issued ID

• Copayment/coinsurance/deductibles – We accept check, credit card (Visa/MC/Discover/AE), or cash.  There is a 4% surcharge for credit cards.  To avoid this, please bring cash, check, or debit card.  Copay, coinsurance, or deductible will be collected upon arrival to our office prior to your appointment with your physical therapist.. 

*Under direct access in Florida, you do not need a physical therapy order from your physician to be seen by a licensed physical therapist for the first 30 days. However, your insurance carrier may require a prescription for coverage of medical claims. Please contact your insurance company regarding your policy prior to your first appointment to verify coverage without a physician referral
